Guiding principle
The site is built to collect the minimum personal information needed to book an appointment and answer enquiries. No detailed health information is requested by the forms on this site.
Information collected
Through the request form: first and last name, telephone, email, where the appointment takes place and its address, the service requested, the preferred date and time of day, whether a medical order is in hand, and an optional practical note.
What the site does not ask for
The form does not ask for detailed medical information, a health insurance number, or a photo of a medical document. Those are handled during the visit or through a validated secure channel.
Use
The information is used only to handle the request, confirm the appointment, send the preparation instructions and carry out the administrative follow-up of the service.
Retention and security
Forms must use appropriate security measures and the site is served over HTTPS. The booking platform chosen must comply with Law 25 on the protection of personal information. Retention periods will be set out once the solution is chosen.
Sharing
No information is sold. Sharing is limited to what the delivery of the service requires, in particular with the partner laboratory when the routing of a specimen calls for it.
Booking platform
Booking is handled by an external platform. What you enter there — including your medical order, when you upload it — is held by that platform under its own terms and the applicable Québec requirements, not by this site.
Where the calendar is displayed inside a page of this site, your browser contacts the platform as soon as the page renders, which passes it your IP address among other things. The name of the platform chosen and a link to its own policy will be published here before launch.
Privacy officer
Law 25 requires that a responsible person be designated and reachable. Their name and contact details will be published here before launch.
Your rights
You may request access to your personal information, its correction or its removal, in line with the requirements applicable to Québec businesses.
Audience measurement
Any audience analytics tool will only be enabled with a configuration that respects the applicable privacy obligations.
